What is Cleritas?
Cleritas is a library of UK clinician-authored consent form templates for medical aesthetic, cosmetic, and adjacent clinical practice. Every template is written by a UK aesthetic physician and medico-legal expert witness. Templates are sold as 4-file bundles (branded interactive PDF + branded editable Word + plain content Word + instructions PDF) under a single-clinic licence.
Why pay for a consent form when free templates exist?
Free and £5 consent templates are typically bullet-point lists assembled by non-clinicians. When a patient complains and your consent form is examined by the GMC, by a coroner, or by counsel, "I downloaded it from Canva" is not a defence. Cleritas templates are structured around the post-Montgomery v Lanarkshire (2015) consent standard, frequency-label material risks, declare off-label use, address brand non-interchangeability, and use section-by-section initialling — the standard UK regulators actually apply when scrutinising consent.
Is Cleritas suitable for nurse prescribers and dentists?
Yes. Templates are written to UK aesthetic clinician standard and are appropriate for any UK-registered prescribing professional regulated by the GMC, NMC, GDC, or GPhC, operating within their scope of practice.
Will Cleritas templates work with Pabau, Jotform, Faces, or Aesthetic Nurse Software?
Yes. Every bundle includes a plain-content Word document — clean clinical text, no branding, no signature blocks — designed to paste directly into any digital consent platform.
Are these consent forms MHRA approved or GMC certified?
No consent template can be regulator-approved, and any product that claims to be is misrepresenting itself. Cleritas templates are clinician-authored documents aligned with UK consent standards. The clinician using them remains professionally responsible for their consent process at their clinic.
What licence applies?
Single-clinic licence. Use within one trading clinic location. Edit, customise, and present as your own clinical document — that is expected. Redistribution, resale, sharing, or use as a template-shop product is not permitted. Multi-site, group, and white-label licences available on request — message before purchase.
Do you offer bespoke fully-customised consent forms?
Yes. A bespoke service — one consent form written entirely from scratch in your clinic's name, with your specific protocol, drug list, and finance terms — is available separately for clinicians who prefer a fully done-for-them route.
Are downloads refundable?
No. In line with the UK Consumer Contracts Regulations 2013, digital downloads are non-refundable once delivered. Pre-purchase queries are welcome by message.
How often are new templates added?
New treatment-specific templates are added weekly. Templates planned across the first launch wave include dermal fillers, hyaluronidase, Profhilo, polynucleotides, microneedling, radiofrequency, lasers, blepharoplasty, ptosis surgery, ectropion repair, brow lift, the full CQC documentation suite, patient intake, treatment record, and photography and marketing consent.
Who is the author?
A UK-based, internationally renowned medical aesthetic physician and medico-legal expert witness with 10 years' experience in medical aesthetics, cosmetic facial surgery, and oculoplastic surgery. The author is kept anonymous on the products by design — the value is the single-author standard, not the personal brand.
